RN Case Manager position at Our Lady of Lourdes Regional Medical Center
Travel position in Lafayette, Louisiana
Contract length: 12 months

Job Requirements

Proof of 2 immunizations (MMR)
Fit Test (N95- 3M 1860, 1860s or 1870+)
Proof of 3 immunizations (Hepatitis B)
Influenza Vaccine
TB Test (QuantiFERON Blood Test or 2-Step TB Skin Test)
Health Statement (signed and dated)
Proof of 2 immunizations (Varicella)
Proof of vaccination (Tdap)
Annual TB Questionnaire (required within 6 months of start)
Resume or Profile

Criminal Background Check Results (FMOLHS)
Social Security Trace (FMOLHS)
OIG Search (FMOLHS)
EPLS (FMOLHS)
National Sex Offender (FMOLHS)

Additional Details

Proof of 2 immunizations, positive titer or Declination accepted.
Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870+. Must be completed within 1 year prior to start. Updated annually.
Proof of 3 immunizations, positive titer or Declination accepted.
Flu Season dates to be communicated each year. Only Medical/Religious exemptions will be accepted with a physician’s signature or written signed statement from traveler.
Proof of negative Blood test or Skin test as described below.
Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
